PARTICIPATION

To Participate In The Programme as a beneficiary, Please follow the FOLLOWING STEPS:.

*Pick your forms at our designated banks nation-wide. The form is N10,000 per woman living with fibroids to be sponsored by religious organisations, associations, corporate bodies, 1st ladies and Governors, LGA chairmen, philanthropist etc to save women living with fibroids.

*Go with the copy of your teller to our outlets nation-wide, pick your forms, referral card, raffle draw tickets.

*Move to our designated clinics/hospitals nearest to your centre on the referral card for your free medical test/scan, free medication pack, free handset and line, free fibroids education handbook
submit 2 copies of your form, then go with a copy, your package and raffle draw ticket to know if you will be among the lucky 1000 people to be selected in your state for the free fibroids treatment.
